Open to anyone with Hydrocephalus 19yrs & Under.
We are looking for one male and one female 'Face of Hydrocephalus' to serve as our National Spokesperson for our 2014 National Hydrocephalus Awareness Campaign, with marketing and promotional materials all through the year ending September 2014 with National Hydrocephalus Awareness Month.
We will randomly select ONE male and ONE female from all entries received.
1) YOU must LIKE this page
2) Just submit one picture of your nominee, between the ages of newborn to 19 years old, with their name, D.O.B., and location.
3) Please include in your email your permission that the PHF is authorized to use the picture for promotional purposes
4) Failure to comply with Rules #1, 2 & 3 will disqualify that entry.
5) Only one entry per child will be accepted. Siblings with Hydrocephalus can be entered separately.
6) ** PHF Board of Directors & State Chapter Directors are eligible to participate and enter their child.
7) All entries will be featured on the PHF website in a pictorial collage.

The 2 winners will be awarded $250.00 donations in their name to your choice of a Hydrocephalus related Research Project we will be reviewing for funding approval, and a 2 Night Complimentary Stay at the Hotel in DC next year for our 4th Annual Hydrocephalus Awareness Day on Capitol Hill. (BOTH winners & their families joined us in DC this past August!)
Contest ends 12/31 & Winners announced on January 2nd, 2014.
